Supporting Documentation

Applications for some types of assistance require additional documentation.

Rental Assistance

Documentation of your rental agreement is required. Please provide a copy of your agreement, including proper name and address of landlord or property management  company. If that is not available a letter from your landlord or rental management company, on their letterhead, validating your rental arrangements is acceptable. 

If you have received a written eviction notice, please include a copy of it with your application if you can.

Utility bills

Water, electricity, or natural gas. Provide a copy of your most recent bill.

Medical Bills

Provide a copy of your most recent bill. 

Minor Car Repair

Provide a written estimate for the cost of the repair, evidence of vehicle ownership (e.g. a copy of your current DMV car registration), and a copy of your driver’s license.


If you can download or create an electronic copy of the bill or cost estimate, your documentation can be sent electronically with your application to intake@wecarecorvallis.org.

If obtaining an electronic version of this documentation is not possible for you, you can place a copy of your bill/cost estimate in our secure drop-box at FCC. If you need us to return an original bill, you have placed in our drop box, please include a note with the document requesting us to return it, and we will send it back to you by mail.
